area, log on to
www.bcbs.com and
click on "Find a Doctor or Hospital" on the right hand side of the
page. Next, click on the link "Blue National Doctor and
Hospital Finder". Enter the prefix "TMJ" under the
Member tab and then click "Find Providers". |

To
find information concerning mental health benefits, log on to
http://www.bhsonline.com and enter
the word "TEAM" in the password field in the middle of the page. |
| Dental Network of America |
To
locate a provider in your area,
log on to
www.dnoa.com
and enter your zip code in the box that says
"Looking for a Network Dentist?" Choose "Labor" from
the dropdown box and hit "go". On the following screen, select
the state in which you reside. |

Log on to
www.eyemedvisioncare.com.
Click on the "Members" tab, then click Login/Register. Once,
you have entered the requested information and created your username
and password, you will have direct access to your vision benefits,
as well as a directory of providers in your area. |
